Person-held Record
A home-based record system supporting independent living.
The FACE Personal Held Record provides a simple, person-centred format for bringing together key documents describing a person's needs and care. Its core aim is to be useful to the service user and increase their sense of control over their daily life. Person-centred care is enhanced by providing the person with immediate access to such details as contact numbers, care plans and timetable, prescriptions and possible medication side-effects.
The Person-held record also enables practitioners supporting the person to access key information on recent contacts and issues, to document their current involvement and keep abreast of the involvement of other practitioners. It thereby enables practitioners to communicate clearly to both the service user and their colleagues, thus helping to ensure co-ordinated care and minimising duplication of effort.
The FACE Person Held Record is designed to be a wholly generic system suitable for any social and health care environment. It is designed to be compatible with locally produced tools, FACE assessment toolsets or even toolsets of our competitors.
The contents of the Person Held Record are being digitised as digital forms and therefore the Person Held Record can be used in combination with our Digital Forms and Pen solution. Using this solution would allow practitioners to leave the paper records with the person while taking the data away for input into an IT system.

Each area is indicated by a simple tab and is accompanied by plain English guidance on the type of documents most suitable to be filed in that area. Other key features include:

Documentation
The Person Held Record is a comprehensive documentation system that has been through a thorough development process. The documentation is pre-inserted into the folder and includes formats for:
- Recording of emergency information, key contacts, appointments, care plans, visits, vital signs, personal directives, useful information etc.
- Supplied list of telephone numbers and web addresses of national organisations.
The documentation is also supplied on CD-ROM so additional copies can be printed as required and local modifications can be made if necessary.

The FACE Personal Records File is the result of a collaboration between Intermation and Clarity Gazebo.
