Single Assessment Process/Older People

Our Overview Assessment was the first assessment tool to be fully accredited by the Department of Health for use in the Single Assessment Process. It is now the mostly widely-used SAP tool. According to Department of Health figures, it is used by 70% of areas using a nationally-accredited tool.

Version 5, released in September 2005 is a truly integrated holistic package. The toolset is designed to provide core documentation for multi-agency work across health and social care for adults and older people. This toolset integrates the processes of assessment and outcomes measurement and also includes care planning and review documentation. As well as the Contact and Overview Assessment the package also includes:

  • The Rapid Assessment, for contexts in which some level of holistic assessment is required but a full Overview is not.
  • A range of 'secondline' tools for more in-depth assessment as required.
  • Care planning and goal-setting documentation.
  • Carer's assessment and Carer's support plan.
  • Service user and Carer questionnaires - simple and straightforward self-assessment questionnaires for completion by service users and carers.

All our tools are supplied in a range of formats and may be fully completed on paper, using Word or as part of a software solution. Our XML schemas are being widely adopted by SAP system suppliers to enable SAP data to be shared acosss health and social care software systems.

A CAF update of the package is due for release in late 2007 and work has already been undertaken to ensure that the toolset meets all CAF requirements.
